Transition Planning Resources for Youth & Families

ConnectABILITY has worked to find several resources that focus on transition planning for youth who have a developmental disability from various stakeholders and organizations across the province.

The Youth in Transition Toolkit, was created by the Southern Network of Specialized Care and is geared towards service providers working with youth between the ages of 16 to 25 who are transitioned from children’s to adult services. Find that here.

The Transition to Adulthood Planning Toolkit, was created by multiple organizations, including two member organizations, Community Living Port Colborne Wainfleet and Community Living Fort Erie. This toolkit helps families plan with their child for success in school, work and life in general.

Last but not least, is the Transition Resource Guide, created for students who have a disability and are heading to college or university. The guide is full of information on programs, support services, accessibility of each school and more.
